Title Clinical Case Manager

Description Make a difference in the lives of children. Join the team of hardworking professionals who help children and families. The ideal candidate will be able to perform a wide range of social service activities as they relate to post adoption practice. Responsibilities include: provide clinical case management services and support to families who have adopted children from the custody of the Texas Department of Family and Protective Services and/or through our adoption program. Assessment of family's needs, provide information and referrals, assist families with establishing needed services (i.e. therapy, psychiatric evaluation, out-of-home placements, etc.), and provide guidance and support to families in effort to maintain intact adoptive families. Monitor and support adoptive families; plan, coordinate, and facilitate quarterly Adoption and Post Adoption Support Group. Some evening and weekend work may be required. THIS IS A 19 HOUR PER WEEK JOB ONLY.
Requirements • Bachelor's Degree in a Social or Human Services Field, or Behavioral Health Science, though a Master's Degree is preferred
• Minimum of two years experience with target population (children and families) or similar professional work; one (1) year of child welfare experience is strongly preferred
• Proficient in Microsoft Office Suite
• Must also be willing to drive throughout the region for home and other field visits
