Come and join the team at Oaks Integrated Care as we open a new program! As an APN, you will work providing outreach to consumers up to the age of 25 in the prodromal phase of psychosis and / or showing other signs of possible psychosis.
Responsibilities:
• Meet with individuals during or within 7 days of intake for medical needs and medication management in clinical settings
• Implement plan of care using psychotherapeutic interventions, holistic health management education, and rehabilitation strategies, and medication and treatment prescribed under joint protocols
• Prescribe psychotropic medications
• Coordinate with the individual’s primary care physician or other health care provider
• Formulate with other team members a plan for treatment and management of physical and mental health problems
• Provide consultation to staff, individuals served, family members, pharmacists and others regarding medication and health issues
• Make diagnosis of acute and chronic health/mental health problems; orders lab and other diagnostic work and interprets results
• Act as a liaison with physicians, psychiatrists and other health care providers to the individual
• Prepare clinical, statistical, billing and personnel documentation thoroughly and on a timely basis
• Attend team meetings, agency sponsored educational meetings, and clinical consultation session regularly
• Other tasks as assigned by Team Leader
