Bridging Family Support provides 1:1 in-home and in-community behavioral, clinical, adaptive, and respite supports for individuals with developmental disabilities and their families. The program is designed to help reduce behaviors that may threaten the health or safety of themselves or others, stabilize behaviors, and increase adaptive, social or functional skills. In-Community Support Professionals assist individuals in safe social, recreational, and community integration activities.

Responsibilities:

  • Provide direct one on one care, supervision, and assistance to individuals using Positive Behavior Supports
  • Support and assist individual in accomplishing daily living skills, positive behavior, socialization, and other identified needs
  • Accompany individual into the community
  • Crisis prevention, recognition, and intervention
  • Staff meeting attendance
  • Ensure all agency policies implemented and followed
  • Complete required trainings
  • Complete progress notes within 24 hours of service provision

Requirements

Qualifications:

• High School diploma

• 6 months experience working with individuals with developmental or intellectual delays

• Valid NJ Driver’s license

• Ability to work days, afternoons, evenings, and/or weekends

All candidates, if hired, will be expected to cooperate with Oaks Integrated Care and The Department of Human Services in all inspections and investigations. All positions require a valid driver’s license in good standing and operating automobile with current automobile insurance, pre-employment drug screening and criminal background check.
