SAMHSA chose New Jersey as one of 8 states to take part in this groundbreaking national initiative. The CCBHC features a new approach to care designed to improve overall health by treating the whole person through community based mental health and addiction treatment. Join our CCBHC team in Mercer County today!


  • Completes phone and face to face screenings for CCBHC;
  • Complete intakes and clinical assessments;
  • Based on assessment, assigning individual to appropriate level of care;
  • Develop Initial Recovery Plan;
  • Collaborate with clinical team;
  • Document all face to face and phone contacts and follow up on referrals;
  • Enter Accurate and timely documentation into electronic health record;
  • Collect intake data and monitor for compliance with funding source standards and overall quality of service provided;
  • Participate in regularly scheduled project management team meetings;
  • Experience using evidence based clinical assessments;
  • On call responsibilities;

  • Master's degree in Social Work or related field (MSW must have LSW);
  • LSW or LAC minimally required; LPC or LCSW preferred;
  • Minimum of one year experience working with children or adults with serious mental illness;
  • Knowledge of evidence-based practices in clinical preventitive services to provide primary healthcare services for persons with serious mental illness;
