Description

The State of New Jersey selected Oaks as one of just 7 providers to participate in a two-year Certified Community Behavioral Health Clinic (CCBHC) demonstration program. SAMHSA chose New Jersey as one of 8 states to take part in this groundbreaking national initiative. The CCBHC features a new approach to care designed to improve overall health by treating the whole person through community based mental health and addiction treatment. Join our CCBHC team in Mercer County today!


Responsibilities:

  • Completes phone and face to face screenings for CCBHC;
  • Complete intakes and clinical assessments;
  • Based on assessment, assigning individual to appropriate level of care;
  • Develop Initial Recovery Plan;
  • Collaborate with clinical team;
  • Document all face to face and phone contacts and follow up on referrals;
  • Enter Accurate and timely documentation into electronic health record;
  • Collect intake data and monitor for compliance with funding source standards and overall quality of service provided;
  • Participate in regularly scheduled project management team meetings;
  • Experience using evidence based clinical assessments;
  • On call responsibilities;

Requirements

Qualifications:

  • Master's degree in Social Work or related field (MSW must have LSW);
  • LSW or LAC minimally required; LPC or LCSW preferred;
  • Minimum of one year experience working with children or adults with serious mental illness;
  • Knowledge of evidence-based practices in clinical preventitive services to provide primary healthcare services for persons with serious mental illness;
