As an LPN, you will work in a residential group home setting for youth with developmental disabilities between various programs in Gloucester, Burlington, and Camden Counties, NJ. These residential programs provides a structured, safe community placement for developmentally disabled children diagnosed with an autism spectrum disorder.
Responsibilities:
• Complete nursing assessment report within 24 hours
• Provide medication monitoring
• Provide emergency evaluation as needed
• Participate in the development and revision of Treatment Plans
• Provide training to staff, consumers, and family members
• Administer medications
• Assess youth condition under direction of the APN and RN
• Provide educational and support to direct care staff on medication administration and side effects
• Provide injections of medication as needed and prescribed by physicians
• Provide weekly health, hygiene, and sex education to youth
• Monitor for accuracy of and confirm physician’s orders
• Coordinate and provide on-going assessments
• Assist in crisis intervention situations as needed
• Face to face health care monitoring
• Collaborate with healthcare providers
• Complete supporting documentation for all of the above duties
• Provide on call support 24 hours for emergencies
