As a Program Coordinator, you will work in an case management setting in Trenton, Mercer County, NJ. The Behavioral Health Home program provides quality services to adults living with a mental illness.

The Program Coordinator is responsible for the day-to-day management of the bahavioral health home (BHH) and for orienting, training and supervising the BHH staff.

Responsibilities:

  • Conducting intakes, physical health and wellness assessments, depression assessments, and suicide assessments.
     
  • Ongoing assessment of vitals
     
  • Assisting in the creation of person-centered plans.
     
  • Providing medication counseling and education.
     
  • Administering injections.
     
  • Providing care coordination and transition of care.
     
  • Providing education for disease management.
     
  • Promoting healthy lifestyle choices.
     
  • Providing linkage to community services.

Requirements

Qualifications:

  • Bachelors or Associates degree in Nursing or related health field;
  • Active, unrestricted NJ license as Registered Nurse;
  • Relevant experience including experience working with persons with mental health disorders required;
