Description

The State of New Jersey selected Oaks as one of just 7 providers to participate in a two-year Certified Community Behavioral Health Clinic (CCBHC) demonstration program. SAMHSA chose New Jersey as one of 8 states to take part in this groundbreaking national initiative. The CCBHC features a new approach to care designed to improve overall health by treating the whole person through community based mental health and addiction treatment. Responsibilities:

  • Oversight of all Intake/Assessment Specialists;
  • Ensures phone and face to face screenings are thorough and complete;
  • Ensures consumers are scheduled for intake as identified by program requirements;
  • Ensures intakes and clinical assessments for consumers are thorough and complete;
  • Ensures that consumers are assigned to appropriate level of care;
  • Complete screening and intakes when necessary;
  • Oversees the completion of Initial Recovery Plan as required by staff;
  • Collaborate with clinical team;
  • Coordinate as needed with the Care Management Team;
  • Community outreach;
  • Ensure documentation compliance for the Intake/Assessment Team in the electronic clinical record;
  • Review and complete program reports as required;
  • Collect intake data and monitor for compliance with funding source standards and overall quality of service provided for the CCBHC;
  • Provide Individual supervision to direct reports;
  • Participate in regularly scheduled project management team meetings;
  • Participate in treatment team meetings;

Requirements

Qualifications:

  • Master’s Degree in Social Work, counseling, or a related field;
  • Clinical License required in the state of NJ (LCSW or LPC);
  • Minimum of 3-5 years experience in working with individuals with serious and persistent mental illness;
  • Two years supervisory or management experience required;
