POSITION SUMMARY:

The ALC Plus Case Coordinator will be responsible for the day to day oversight and management of assisting enrolled ALC Plus high school students to achieve success in reaching educational goals. The ALC Plus Case Coordinator will consult with the education team, system navigator, school social workers, Hennepin County staff, and parents to ensure program success. Position will support student with school attendance, academic and behavior goals, as well as support connecting students to mental and chemical health services as needed.

 

JOB RESPONSIBILITIES:

  • Promote positive social-emotional skills with students while offering guidance and support to students and their families.
  • Consult and collaborate effectively with parents/guardians, educational staff, and community professionals regarding student progress.
  • Monitor students’ academic, behavioral, and attendance progress, developing interventions as needed. • Participate in recreational and educational after-school activities for students.
  • Transport youth as needed to services and community resources between home, school, and resource locations.
  • Track data on students around outcomes and progress towards goal completion. Prepare reports as needed.
  • Maintain ongoing case notes documenting contact with student, family and community supports.
  • Participate in the intake process as students enroll in school and upon enrollment identifying unmet needs of students.
  • Participate in school staff meetings, family conferences, school-wide events, and school activities.
  • Assure program participant privacy by being knowledgeable and following HIPPA/FERPA data privacy and security regulations.
  • Complete and submit expense documentation in a timely fashion.
Requirements

REQUIRED QUALIFICATIONS:

Education and Experience

  • BA degree in Human Services, Social Work, or related field.
  • Two years’ experience in working with youth and families.
  • Knowledge of educational systems, Hennepin County Services, and community resources.

      Knowledge, Skills, and Abilities:

  • Ability to communicate effectively, both orally and in writing.
  • Ability to work with diverse populations in challenging situations.
  • Ability to develop healthy relationships with students and families.
  • Ability to have a flexible schedule to accommodate meetings, phone calls and email inquiries.
  • Knowledge of adolescent chemical and mental health issues.
  • Ability to travel to various locations for meetings and home visits.

Licenses and/or Certifications:

  • Must have a valid MN driver’s license and insured vehicle for transporting students and families.
